Senior Services at Senior
Care Institute: During the medical assessment, the geriatrician physically examines the patient. The doctor considers the patients physical and medical problems, health practices, and daily functioning. A social worker talks to the patient and family members to get an understanding of the patients social and environmental situation, individual and family background, and how the patient interacts at home and in the community. During a nursing assessment, a nurse discusses safety, medication use, and health education with the patient. The nurse also may check the patients ability to hear, see, and move. When appropriate, a geriatric psychiatrist or psychologist will join the team in assessing problems such as depression, confusion, or memory loss. After the assessment, members of the treatment team work together to create a plan for the patients continuing care. For patients who are involved with or need specialists not affiliated with the institute, Senior Care Institute physicians and staff attempt to coordinate medical care so that the appropriate health professionals are involved and aware of medical concerns. For patients who receive only an evaluation at the Senior Care Institute, a complete medical summary and treatment recommendations are sent to the physician identified by the patient and family as the one most responsible for continuing primary care.
