Our Services

We care for the entire family, and provide these services:

care for children and adolescents, including well-child checkups, immunizations, and sick visits
family planning and pregnancy care, including delivery
women’s health care, including Pap tests, colposcopy, endometrial biopsy, and infertility evaluation
total adult health care and health screening, including sigmoidoscopy
care for older adults, including nursing home care
care for emotional, marital, and social problems
sports medicine and treatment of minor injuries, including fractures
treatment of minor skin problems, including minor skin surgery and stitches
house calls in the area if needed for your condition
nurse visits (such as blood pressure checks and completion of WIC forms)
laboratory services, including blood draws, throat and urine cultures, and blood sugar tests
radiology services
24-hour-a-day physician availability

Medical forms and records
Often, parents must arrange for completion of multiple physical exam forms for their children for sports or camp physicals during a single year. If your child has been to the office for a regular well-child exam within the last year, we will be able to fill out most forms without another office visit. Drop the form off at the office or mail it to us at:

Your doctor will complete the form based on the previous examination. Allow about a week for us to complete the form for you. All disability, workers’ compensation, premarital, or driver’s license forms require an office visit.

If you require a copy of your medical records, please be aware that your medical records contain confidential information that we release only with your written consent. Please allow sufficient time to process your request (typically two weeks).
